If they need us, the dedicated staff at the Long Island State Veterans Home stands ready to provide skilled nursing care, adult day health care and short term rehabilitation to the 85,000 Veterans who reside on Long Island. We are proud to care for America’s Heroes – those brave men and women who have helped to make our nation secure and have advanced the cause of freedom throughout the world.
Our team strives to bring the most advanced medical technology and best practices to care for our Heroes. This has helped us earn the reputation as a leader among the nation’s 171 State Veterans Homes as well as private long term care and rehabilitation care providers. Additionally, our veterans truly benefit from our relationship with Stony Brook Medicine. As one of the few “teaching nursing homes” in the United States, we are proud of our mission to train the next generation of physicians, nurses, therapists, dieticians, social workers and administrators as it relates to the care of those who need skilled nursing and/or post acute rehabilitation. We strongly believe that no other skilled nursing facility on Long Island can match us as it relates to quality, cost and culture in caring for veterans and their families.
The Long Island State Veterans Home provides financial benefits to our Veterans that they do not receive in other skilled nursing settings. We continue to advocate for our veterans both in Washington, D.C. and Albany and recent federal legislation now provides those veterans who are 70% or more service connected disabled to receive no cost nursing care and medical model adult day health care in any State Veterans Home. With the average cost of nursing home care on Long Island exceeding $150,000 per year, Veterans can access their VA entitlements and utilize them at our home, thereby substantially reducing the financial burden of long term care. Eligibility
The applicant must require skilled nursing care and:
- Was a resident of New York State at the time of entry into active duty or be a New York State resident for one year prior to application for admission; and
- Veteran must also have had an honorable discharge from the U.S. Armed Forces; and
- Veteran must have had at least 30 days of active service other than “active duty for training” (ACDUTRA).
- A spouse or widow of an otherwise qualified Veteran, unless legally separated, may be admitted to a home if the spouse was married to an eligible Veteran for one year prior to application and requires skilled nursing care.
- Gold Star parent – a mother or father of a member of the U.S. Armed Forces who died while on active duty.
Adult Day Health Care
Caring for a physically or cognitively impaired family member at home, 24-hours per day, 7-days per week, can be one of life’s most difficult challenges. Whether you are a spouse or a caregiver, the Adult Day Health Care at the Long Island State Veterans Home (LISVH) may be a welcome solution. Participants at the Adult Day Health Care program receive the skilled nursing care they require while enabling them to continue to live at home. Adult Day Health Care permits caregivers to meet professional and family obligations or take a well deserved rest while their loved one attends the program.
Adult Day Health Care at the LISVH is designed to promote socialization, stimulation and maximize the participant’s independence while enhancing quality of life. The LISVH’s medical model Adult Day Health Care program provides comprehensive medical, nursing and personal care services combined with engaging social activities for physically or cognitively impaired adults. Our program is staffed by a caring and compassionate team of multi-disciplinary healthcare professionals who evaluate each participant and customize an individualized plan of care specific to their health and social needs. We are committed to caring for our nation’s heroes with the dignity and respect they deserve.
Our Adult Day Health Care program provides our participants with:
- Door through Door transportation
- Delicious and nutritious meals
- Care for the body, mind and soul: Medical, nursing, rehabilitation, psychosocial, recreational, nutritional, spiritual and personal needs
- Comprehensive medical, nursing and personal care services: Skilled nursing care, monitoring of health conditions, and administering of medications by our licensed nursing staff
- Assistance with personal hygiene, including bathing and grooming
The LISVH Adult Day Health Care program accepts honorably discharged veterans from all branches of the United States Armed Forces. Admission to the LISVH Adult Day Health Care program is open to all veterans regardless of period of service (war or peace time service), service connected disability or location in which the veteran served our nation. The LISVH Adult Day Health Care program also accepts the spouse or widow of qualifying veterans, as well as Gold Star Parents, who require adult day health care.

Skilled Nursing Facility
The LISVH is a 350 bed skilled nursing facility located on 25 wooded acres on the campus of Stony Brook University. An Adult Day Health Care program that operates 6 days per week (Monday through Saturday) is also offered.
The LISVH is a major teaching affiliate of Stony Brook Medicine and continues to be one of the only skilled nursing centers and adult day health care programs in the country that is fully integrated into the health and educational mission of a major teaching and research university. The programs at the LISVH are licensed by the New York State Department of Health and the U.S. Department of Veterans Affairs.
The LISVH accepts honorably discharged veterans from all branches of the United States Armed Forces. Admission to the Long Island State Veterans Home and the Adult Day Health Care Program is open to all veterans regardless of period of service (war or peace time service), service connected disability or location in which you served our nation. The LISVH also accepts the spouse or widow of qualifying veterans, as well as Gold Star Parents who require skilled nursing or adult day health care.
